This assessment supports applications for SIL funding and includes evaluation of a participant's functional ability, support needs, and goals for living independently (with or without support).

  • Assessment of daily living skills
  • Recommendation of roster of care
  • Collaboration with providers and stakeholders

For participants requiring specialised housing due to extreme functional impairment or high support needs. This assessment provides functional evidence to support SDA eligibility and the most suitable housing model.

  • SDA-specific functional analysis
  • Collaboration with housing providers, support coordinators, and therapists

We assess the need for equipment that improves safety, independence, and access to daily tasks.

This may include:
